DATA CONTROLLER APPLICATION FORM

  1. General Information

The Law No. 6698 on the Protection of Personal Data (“KVKK”) grants personal data owners (“Applicant”), defined as related persons, the right to make certain requests regarding the processing of their personal data in accordance with Article 11 of the KVKK.

This application form has been prepared to identify your relationship with Compro Bilişim Teknolojileri A.Ş. (“Compro”), to fully determine your personal data, if any, processed by Compro, and to provide an accurate response to your application within the legal timeframe. To ensure the security of your personal data and prevent unlawful data transfer, Compro may request additional information for identity and authorization verification. The Applicant is responsible for the accuracy and/or currency of the information provided by the Applicant or if the requests are unauthorized.

In accordance with Article 7 of the Communiqué on the Procedures and Principles of Application to the Data Controller, no fee will be charged for up to ten pages if the Applicant’s application is answered in writing. A transaction fee of 1 Turkish Lira may be charged for each page exceeding ten pages. If the response to the application is provided on a recording medium such as a CD or flash memory, a fee may be charged for the cost of the recording medium.

  1. Scope of the Right to Apply Under Article 11 of the KVKK

By applying to Compro, the Applicant may request the following:

1) To learn whether their personal data has been processed,

2) If their personal data has been processed, to request information regarding this,

3) To learn the purpose of processing their personal data and whether they are used in accordance with their purpose,

4) To learn the third parties to whom their personal data is transferred domestically or abroad,

5) To request correction of their personal data if it is incomplete or incorrectly processed, and to request notification of the transaction made within this scope to third parties to whom the personal data has been transferred,

6) To request the deletion, destruction, or anonymization of their personal data if the reasons requiring processing have disappeared, despite the fact that it has been processed in accordance with the KVKK and other relevant legal provisions, and to request notification of the transaction made within this scope to third parties to whom the personal data has been transferred,

7) To object to a result against them that arises through the analysis of the processed data exclusively through automated systems,

8) To demand compensation for the damage in case of damage due to the unlawful processing of their personal data.

  1. Application Method

In accordance with the first paragraph of Article 13 of the KVKK, applications regarding these rights must be submitted to us in writing and signed or by other methods determined by the Personal Data Protection Board (“Board”).

In this context, written applications will be made by printing out this form;

  • With the Applicant’s personal application by hand,
  • Through a notary public,
  • By registered electronic mail (KEP) address, secure electronic signature, mobile signature, or
  • It can be sent to us by e-mail from the e-mail address previously notified to us by the Applicant and registered in our system.

Compro may request additional information to verify the identity of the Applicant in applications to be made by the Applicant via e-mail, and may take the necessary measures.

In accordance with the second paragraph of Article 13 of the KVKK, your applications submitted to us will be answered in writing or electronically within thirty days from the date of notification or receipt of your request, depending on the nature of the request.
